There is a lot of education that the patient requires and needs to successfully manage their new illness. The goal should be to reach their highest level of wellness, and possibly reverse their condition. Providing all the required information at once can be overwhelming so it is important to set up a series of appointments and provide referrals to nutritionists for example to help the patient to learn and decrease their risk of atherosclerosis and cardiovascular diseases.

    Initial medications that this patient can take in combination with lifestyle and diet changes will help to protect her wellness and through proper management achieve her goal of not requiring insulin injections. A first line drug that works well is metformin extended release 500mg by mouth twice daily (Micromedex, 2022). The brand name for this medication is Glucophage and its class is a biguanide. This is the first line treatment recommendation for new onset diabetics. The risks of this medication should be discussed with the patient which include diarrhea, gas, nausea, vomiting, indigestion, abdominal discomfort, anorexia, headache, rash, and ovulation reduction (Micromedex, 2022). It is important to explain this medication works by reducing the amount of hepatic glucose production and intestinal absorption and increasing the sensitivity to insulin itself (Micromedex, 2022).

    When the patient returns in the following month with complaints of fatigue, dry hair and nails, constipation, and heavy irregular menstruation it is important to order the necessary lab work to create a differential diagnosis. Hyperthyroidism would not be suspected here as it typically presents with increased and or irregular heartbeat, palpitations, weight loss despite increased appetite, nervousness, anxiety, and or irritability (Hyperthyroidism – Symptoms and Causes – Mayo Clinic, 2020). The lab work that should be ordered should include a TSH, T4,T3, TGB, Auto-antibodies, thyroid ultrasound, ECG, and VBG (Hypothyroidism, 2019). Hyperthyroid would show low TSH High T3 and T4 (Hypothyroidism, 2019, Table 4.2). Hypothyroidism is the exact opposite. Initial treatment for hypothyroidism should be levothyroxine which should be started at a concentration of 1.7-15 mcg/kg/day until therapeutic dose is found (Micromedex, 2022). The mechanism of action for this medication is synthetic thyroid hormone (T4). This medication is known by brand name as Synthroid, it should be taken by mouth once daily. It is important to explain to the patient that this medication must be taken exactly as directed and that changes in health should be immediately reported, around .1 percent of the population especially older adults can have the adverse effect called myxedema coma (Hypothyroidism, 2019). They should watch for symptoms as should their family that include feeling cold and a body temperature of 95.9 or less, bradycardia or slow heart rate, low blood pressure, decreased respiratory rate, upper airway obstruction, unusual behavior, or coma (Hypothyroidism – Symptoms and Causes – Mayo Clinic, 2020).

    It is important to continue to order serial lab work and patient follow-up appointments. Monitoring both the patients A1C and thyroid levels to determine that therapies are effective. The patient should be asked if they have continuing symptoms or new symptoms to determine if therapeutic levels have been achieved. Caring for people with multiple comorbidities is important to understand that they are new to these processes and treatments, and continued appointments helps to determine medication compliance and effectiveness.

We refer to healthcare as systems as they are meant to be setup in a way that they keep on churning the desired results. System in general can be broken down into smaller foundational blocks. Traditionally, if the healthcare system was failing to deliver on the key objectives, then the foundational blocks or services were analyzed individually and optimized for better results (Lipsitz, 2012). Looking at healthcare as a complex adaptive system will help in devising a more effective strategy given the ever-evolving nature of the interactions that take place within them.

    Leadership can play a pivotal role in helping healthcare organizations scale and achieve their objectives. Leadership concepts can help leaders in relaying the vision and organizational objectives to the workforce that can help them form an effective strategy to deliver results. Complex adaptive systems has a core focus on cultivating leadership skills by promoting collaboration (Pype, 2018). The top-down approach doesn’t always serve the organization well. Process improvement is a key aspect of complex adaptive systems and leadership can help in making the workforce subject matter experts in the key processes. Being agile is important in the ever-changing healthcare organizational landscape. We have seen some of the well evolved healthcare services and systems crash during the pandemic (Sturmberg, 2021). This was largely due to the rigid nature of the systems and lack of leadership in making them more agile. Due to that, the system was unprepared to adapt, and the leadership of those organizations failed to devise a compelling strategy. Leadership should set up clear milestone and reward innovative approaches for meeting those milestones and goals.

    Jayesh Mahesh Bellara Discussion Week 1COLLAPSEIn the simplest form, Complex Adaptive Systems (CAS) can be defined as a way of thinking and analyzing things based on their complexity, patterns, and inter-relationships rather than focusing on the cause and effect. Since this is an early definition, there have been many changes since this term was coined. The most recent and accepted definition of CAS is “complex and patterned output arising from simple, fundamental principles, but requires many actors and multiple interactions over time to produce the emergent complexity. Yet, this characterization represents only a sample of the myriad of different facets of complexity” (Dodder R, 2000). A CAS may be sensitive to specific small changes in initial conditions. An apparently trivial difference in the beginning state of the system may result in enormously different outcomes. This phenomenon is sometimes called the “butterfly effect.” However, this sensitivity has to do with the complex system’s exact path into the future rather than its general pattern. CAS, including the weather, tends to maintain generally bounded behavior, sometimes called an “attractor,” regardless of small changes in initial conditions. As a result, CAS is robust or fit. They exhibit the ability to alter themselves in response to feedback. Complex systems possess a range of coupling patterns, from tight to loose (Marion, R., 2000). These different patterns help organizations survive a variety of environmental conditions. CAS has been used to inform thinking about leadership and organizational development in various sectors outside of healthcare services. These sectors are economics, engineering, urban planning, engineering, management, etc. (Burns, L., 2011).
    Healthcare remains the major field where CAS is applied extensively for its day-to-day management. Significant areas of focus in healthcare management are organizational development, workforce development, service delivery, behavior change, clinical analysis, and leadership management. A US hospital used complex adaptive systems thinking to grow organizational capacity and cope with unexpected surges in demand for services. Seventeen improvement projects were implemented over two years. Using a complex adaptive systems approach, improvement ideas ’emerged’ from the microsystem at the point of care. Rigorous reporting and testing of process adaptations and staff motivation and interactions drove innovation. Hundreds of clinical and administrative staff at all levels were encouraged to redesign processes and roles to increase organizational capacity. Capacity increased, and this approach was cost-effective. The authors concluded that complex adaptive systems thinking could encourage multiple, coordinated, and concurrent projects that will create a more significant impact than possible with a single improvement project (MacKenzie R, 2008). Researchers in England used complex adaptive systems thinking to explore why the effect of a single intervention incorporated into a complex clinical environment may be different from what is expected. Demographic and health information was collected from all patients admitted to a UK hospital between January 2003 and June 2004. Using charts, continuous process monitoring was undertaken to detect planned or unplanned organizational process changes affecting mortality outcomes (Cockings JG, 2006). These examples show that complex adaptive systems thinking has been used increasingly frequently to analyze development and delivery, organizational planning, and research development tools in healthcare, with favorable results.
    Since around 2000, complex leadership (CL) has been applied in health care management and organization. Leadership is regarded as behavior or set of behaviors that emerge from the interaction among individuals and groups in organizations occurring throughout the organization, not a role or function formally assigned to an individual. However, CL and organizational performance in healthcare are not as developed as some of the other fields. Many researchers emphasized the need to pay closer attention to the quality and nature of leadership in the field of healthcare in order to have lasting effectiveness (Belrhiti, Z.,2018). A survey conducted by Burns JP in 2001 found that healthcare leaders intuitively support the principles of complexity science. The authors argue that building on complex adaptive systems thinking principles can help leaders focus less on prediction and control and more on fostering relationships and creating conditions so systems can evolve and produce creative outcomes (Burns JP, 2001).

    Using the outline and the resources and information from the annotated bibliography, compose a final research paper. This paper should be focused on public health and include biostatistics-related content, including information related to the following elements learned:
    population and sample size;
    hypothesis testing;

Such a realization on the part of your instructor invariably leads to a precipitous drop in the paper grade.
    3.The word novel should only be used to refer to a work of fiction. Do not refer to a piece of academic writing or non-fiction as ‘a novel’.

    4.Italicize book titles, journal titles, newspaper titles, DVD/movie titles, documentary titles, etc. Only use quotation marks around the title of an article. Ex: John Smith, “How Writing History Papers Saved My Life and Brought Me Inner Peace,” New Academic 1:5, 46-112.
    5.Beware of shifting verb tenses (especially in the same sentence!). As a general rule, it is easiest to stay in past tense when you are writing history papers. The exception is when you are directly referencing an author’s work (even when they are dead). Example: “In his work The Interpretation of Dreams, Freud says….”
    6.Please keep in mind the following pitfalls; there are others, but these are the most frequent I have encountered over the years:

    Be consistent with your capitalization. Note: Socialism, Capitalism, Nationalism should always be capitalized; socialists, capitalists, nationalists should not be.

    8.Proofread! Spellcheck is not enough. I highly recommend reading your papers out loud after you have written them. It is an excellent way to make sure that your writing is clear and makes sense. If you read it out loud and a sentence sounds strange to your ear, there is most likely something wrong with it. Figure out what needs fixing and fix it.

    Bibliography and Citation

    Your paper must have proper footnotes / endnotes, and a formal bibliography. These must follow the rules of either Chicago Manual of Style/Kate Turabian or MLA style.
