Care Plan is based off Nursing Diagnosis located at the top of form. THERE ARE TWO SEPERATE nursing diagnosis, a care plan form will need to be completed for each diagnosis. The other documents are an assessment that was taken to help create care plan. Please use that as assistance. The template MUST be used to fill out.
