l. What additional information would you ask the nurse to provide at this time?
Ask the questions: “what about his lung sounds and pulse oximeter readings?”, “how is he tolerating the reduced lung volume?”, “does he have any allergies?”, “does he have any family member who will come to see him?”, “has he been eating, and what type of diet does he follow?”, “what are his elimination patterns – has he voided and made bowel movements?”, “how are his I/O?”.
2. What issues or problems can you already identify?
The identified issues/problems are the following: a. Safety issues – He just had surgery on the right side of his chest 3 days ago, as well as he is sitting on the edge of the bed and the side rails are set down. If he loses his balance, he would not be able to use his right arm to help in stabilizing himself; b. Pain problem – He is already on the 3rd day of postoperative period but he has not yet received anything for his complaints of pain at the insertion site; c. Chest tube dislodgement risk – He has a right midaxillary chest tube to Pleur-Evac drain, but he keeps rubbing his left hand over the right side of his chest; d. Sleep issue – He has not yet slept since the lobectomy 3 days ago.
3. Describe four things you need to do right now for D.S.
Actions to take: a. Raise the right-side rails and explain that this measure is for his safety. You cannot raise all side rails because doing so is considered as a restraint; b. Explain that your rooms are clustered at the end of this hallway so that you can keep an eye on all your patients. This can reduce his sense of isolation; c. Obtain his vital signs and SaO2 to monitor his condition; d. Perform a focused assessment to have detailed nursing assessment of the patient’s current problems/concerns; d. Evaluate lung sounds, chest tube site, and IV site, as well as all of the peripheral materials/equipment, including oxygen delivery equipment, chest tube, and IV materials for monitoring and ascertainment of problems/issues; e. Acknowledge that his chest tube is causing pain. Ask him to describe the pain, particularly: whether the pain is sharp/dull, constant/intermittent, or localized/generalized, whether the pain stays at one site or radiates to somewhere else, and what relieves it and what makes it worse – in order to have accurate assessment and treatment plan; f. Investigate the reason why he has refused to take pain medication since surgery to determine what actions to take; g. Give him the call light so that you can respond right away if he needs help or if he has important concerns.
4. How would you respond to D.S.’s statement?
-Teach D.S. about the fact that the presence of pain signifies that something is wrong with his body which helps to determine when to seek medical attention. Pain medication is ordered to relieve pain so that he can move around, perform appropriate activities, cough and breathe, and recover faster. Moving and coughing will prevent fluid from accumulating in his good lung in order to avoid having pneumonia.
-Inform D.S. that patients who take pain medication have fewer complications and recover quickly. Explain that if he waits until the pain is intense, it will take larger doses/amounts of pain medication to relieve the pain. Teach him about the fact that his body’s response to pain can delay healing.
-Taking pain medication after surgery is needed to relieve the soreness. It is only taken when needed. Tell him that he needs to cooperate so that he can recover and be discharged early.
-Discuss both pharmacological and nonpharmacological methods of pain relief.
5. Which of the following nonpharmacologic methods of pain relief would most likely help D.S.at this time? Select all that apply.
a. Distraction b. Hypnosis c. Positioning d. Acupuncture e. Biofeedback
Nonpharmacological methods can help in reducing pain.
6. D.S. is experiencing difficulty using his right arm. Given the type of surgery he underwent, is this expected and why?
It is an expected outcome of this type of surgery. The standard posterolateral thoracotomy incision for lobectomy starts along the inframammary crease and extends posterolaterally below the tip of the scapula; it is then extended superiorly between the spine and the edge of the scapula (a short distance); afterwards, the trapezius muscle and the subcutaneous tissues are divided; and then, the intercostal muscles are divided along the superior border of the ribs, and the thoracic cavity is accessed. Postoperatively, the patient suffers a transient nerve compression and restricted use of the arm on the affected side.
7. You administer morphine sulfate 4mg IV and tell D.S. that you will return in 30 minutes; 15 minutes later, he turns on his call light. When you enter the room, D.S. says, “I think I’m going to throw up.” What are the next three things you would do?
The next things to do would be to: turn him onto his side to reduce the risk for aspiration; grab a basin and place it beside his mouth; provide a cool washcloth to his head; and ask him when he began to feel nauseated.
8. D.S. states, “I started to feel sick a couple minutes ago. It just kept getting worse until I knew I was going to throw up.” What do you think is responsible for the sudden onset of nausea?
Nausea is one of the adverse reactions associated with morphine.
9. Would it be appropriate to give D.S. a second dose of morphine? State your rational
It would not be appropriate to give D.S. a 2nd dose of morphine since there are other alternative medications that the physician could order after the patient had adverse reaction to morphine. Call the physician and describe your findings/observations during your initial assessment. Describe the patient’s reaction to the morphine sulfate and ask if he can try fentanyl or hydrocodone.
