Actions to take: a. Raise the right-side rails and explain that this measure is for his safety. You cannot raise all side rails because doing so is considered as a restraint; b. Explain that your rooms are clustered at the end of this hallway so that you can keep an eye on all your patients. This can reduce his sense of isolation; c. Obtain his vital signs and SaO2 to monitor his condition; d. Perform a focused assessment to have detailed nursing assessment of the patient’s current problems/concerns; d. Evaluate lung sounds, chest tube site, and IV site, as well as all of the peripheral materials/equipment, including oxygen delivery equipment, chest tube, and IV materials for monitoring and ascertainment of problems/issues; e. Acknowledge that his chest tube is causing pain. Ask him to describe the pain, particularly: whether the pain is sharp/dull, constant/intermittent, or localized/generalized, whether the pain stays at one site or radiates to somewhere else, and what relieves it and what makes it worse – in order to have accurate assessment and treatment plan; f. Investigate the reason why he has refused to take pain medication since surgery to determine what actions to take; g. Give him the call light so that you can respond right away if he needs help or if he has important concerns.
4. How would you respond to D.S.’s statement?
-Teach D.S. about the fact that the presence of pain signifies that something is wrong with his body which helps to determine when to seek medical attention. Pain medication is ordered to relieve pain so that he can move around, perform appropriate activities, cough and breathe, and recover faster. Moving and coughing will prevent fluid from accumulating in his good lung in order to avoid having pneumonia.
-Inform D.S. that patients who take pain medication have fewer complications and recover quickly. Explain that if he waits until the pain is intense, it will take larger doses/amounts of pain medication to relieve the pain. Teach him about the fact that his body’s response to pain can delay healing.
-Taking pain medication after surgery is needed to relieve the soreness. It is only taken when needed. Tell him that he needs to cooperate so that he can recover and be discharged early.
-Discuss both pharmacological and nonpharmacological methods of pain relief.
