Se trata de un paciente masculino de 74 años de edad, soltero, instrucción primaria, ocupación agricultora, católico y lateralidad zurda. El paciente acudió al servicio de neuro-rehabilitación porque demanda deterioro del estado de conciencia.
Se reporta que, a mediados del 2019, el sujeto sufrió una riña mientras se encontraba consumiendo alcohol en la calle, producto de lo cual sufrió un grave traumatismo craneoencefálico. Los servicios de emergencia acudieron a su atención, encontrándose al paciente con signos de hipotermia[1], hematoma (golpe) en la hemicara derecha, alteración de su conciencia y pérdida de lenguaje. Se le trasladó a un hospital local donde le realizaron una tomografía axial computarizada del encéfalo, encontrando hematoma en el hemisferio derecha. En la evaluación de neurocirugía se detectó un traumatismo craneoencefálico moderado, crisis convulsivas y se consideró que no requería intervención neuroquirúrgica alguna.
En la valoración neurológica se reporta que durante la entrevista se observó tranquilo, colaborador y abordable, se identifica hemiparesia[2] de hemicuerpo izquierdo, orientado en persona, desorientado en tiempo y espacio, juicio y razonamiento disminuido, lenguaje poco fluente, moderadamente organizado en forma y contenido, se requiere corroboración de la información con el hermano durante la entrevista, ya que, presenta un relato confuso y por momentos olvida los hechos vinculados al accidente, además, es confuso e inentendible su relato de anécdotas. Logra denominar, repite palabras, comprende órdenes simples (ven, guarda, siéntate), mas no comprende órdenes complejas (ordenes que incluyen dos a más acciones).
En cuanto a la evaluación neuropsicológica se reporta el paciente presenta dificultades en el cumplimiento de las normas del hogar en el que está hospedado, pese a que las mismas han sido explicadas en reiteradas ocasiones, aún no puede adecuarse a ellas.
En cuanto a su conducta en el medio familiar, el paciente no tiene una lucidez para darse cuenta de los errores que comete y al recibir los reclamos de sus familiares por la conducta inapropiada del paciente, como, por ejemplo, ocupar de manera incorrecta el baño, no limpiar su espacio y rechazar cualquier tipo de ayuda que se le ofrece, ante lo cual el paciente reclama no necesitarlo.

Asimismo, se identificaron los siguientes síntomas: dificultades graves en procesos perceptivos visoespaciales y viso constructivos, déficit en la supervisión de la actividad, en la planificación viso espacial y en las capacidades de control motriz.
