The grant proposal that is chose for my capstone project is an issue that stays at the top of the list for grant funds. Maternal and child health. Human Resources and Services Administration (HRSA) Maternal and Child ild Health has a history of offering grants to support communities of high risk for poor health outcomes. The Title V Maternal and Child health block grant has several recipients who have been successful with implementing programs that help improve the health of women and children. The Maternal Infant Services Network are the recipients of serval grants and has successfully maintained the grant deliverable to offer services is to the target population and communities that they serve.

The Maternal Infant Child Health Collaborative (MICHC) is one of the grants that the agency has that includes a Community Health Worker (CHW) program that provides home visiting to women. One weakness of the program is staff retention. The roles of a CHW requires candidates that live in the community and is able to identify and form collaborations with other community agencies. With a high staff turnover, the relationship with community partners and clients are not sustained. The design element that can inform my capstone project is the sustainability of the community health worker program and the reasons for its success. As on of the only CHW programs that serves tow counties in the area with increases in programs that are created to support women, children, teens. This information will help me as a guard as I develop my capstone project,
