Services
Oakley Care allows us to help you with care you need both in basic support and intensive support services. Minnesota Statutes, Chapter 245D, regulates the provision of home and community-based services to persons with disabilities and persons age 65 and older.
245D Services can assist you in obtaining or regaining independence in your home and in the community. Our highly qualified caregiver will assist you in achieving your goals and objectives by first learning about what is important in your life and then devising a strategy to get you there. Our team keeps you connected to your community and assists you in completing significant tasks in your life. Bringing our caregivers into your life restores your sense of peace.
Basic
24-Hour emergency assistance- Our staffs provide on-call counseling and problem-solving, as well as quick support at a person’s house in the event of a medical or personal emergency.
Adult companion services – Our companion services offer emotional support and non-medical monitoring to improve socializing experiences and improve your life.
Homemaker services- When you are unable to do so, our experienced Homemakers may assist you with general cleaning of your house.
Night supervision- Offers nighttime support and surveillance inside the person’s home by awake personnel.
Personal support- Services are offered in a person’s home or community to assist them in reaching their full potential, improving their independence, and attaining their community participation goals.
Respite care services- We provide short-term care services to guarantee your health
