Dissociative Disorders Readings #1 and #2
There are different Dissociative Disorders. These include Dissociative Identity Disorder, Dissociative Amnesia, and Dissociative Fugue.
Dissociative Disorders involve changes in one’s sense of self, perceptions, and memory. See Reading #1 for a detailed definition of this term.
Let’s start with Dissociative Identity Disorder (DID). This disorder used to be called Multiple Personality Disorder in previous DSM editions. What you have here is an individual who has their original or “core” personality and who then develops new alternate personalities (some patients report and manifest up to 100 or more!).
These are separate and distinct personalities that are completely different from the core. They may be of either gender or of different ages. So, for instance, a 34-year-old with DID could manifest a 5-year-old child personality. Sometimes, there are physical changes associated with the manifestation of these personalities (e.g., eye color may change).
Also, when an alternate personality manifests itself the core “loses time,” meaning that when the core returns there is memory loss for that time period. So, for instance, an individual with this illness could find themselves at the mall and have no idea how they got there or why. One of their alters may be a “shopper’ who took control and went shopping. The hours shopping were lost to time for the core.
This disorder is highly linked to past physical and or sexual abuse. It is theorized that the human mind develops different personalities in response to such abuse as a means of coping. For instance, a tough personality might develop to handle different forms of life stress.
Those with this disorder will sometimes report that they can hear the personalities speak in their minds. Some report that they have named each and yet others report that they have not named all of them. Some also report that some of the personalities don’t get along! There does not seem to be any hard and fast rules with regard to this phenomenon.

Now, there has been controversy within the field of mental health regarding this illness. Not everyone agrees that it is a true condition. There are some who believe that it could be part of another disorder (e.g., psychotic disorder), or that individuals could be faking such symptoms.
Dissociative Amnesia-This disorder involves memory failures for past events. It may occur in response to a traumatic event. The amnesia is not due to head trauma or substance abuse. It is psychogenic, meaning that it is caused by psychological factors—at some level we wish not to remember the events(s). There are different forms of this illness. They are:
Systematized-This involves memory loss for a specific category of information (e.g., one’s siblings).
Continuous-This involves not being able to make new memories. Events are forgot as they happen.
Localized-This is the most common form. It involves not being able to remember data from a specific time period.
Generalized-This involves a total loss of memory for one’s own life.
Selective-This involves being able to remember some data from a particular time period but not all.
Now, some people with amnesia develop Dissociative Fugue. A fugue state involves unexpected and unplanned travel, whereby an individual will literally leave where they are and travel to another location. During such a state, individuals are amnestic for their past (this means they don’t remember their name or where they are from). There have been cases where people with this illness have moved and created a whole new identity and life—but this is rare. Typically, memory returns and so does the person! Fugue states are typically the result of extreme stress with the moving and memory loss being seen as a means to essentially forget one’s troubles.

Depersonalization/Derealization Disorder-This is a disorder in which individuals experience alterations in their perceptions of self and world. They may feel as if they are living in a dream-like state. Make sure to read the details!
As indicated in Reading #2 the Posttraumatic Model (PTM) suggests a direct causal link between traumatic life experiences and the development of dissociative states. However, there are some difficulties with this explanation. Make sure that you read/study that section of Reading #2.

Somatic Symptom and Related Disorders
As a prefix “soma” means body. These are disorders which take a bodily form.
Let’s start with Somatic Symptom Disorder.
This disorder involves the experience of numerous somatic complaints, often pain, that may or may not be explained by a physical cause. Regardless, the individual with this illness experiences true suffering and is in need of care. They are very distressed about their symptoms and they impact their functioning (e.g., going to work).

Illness Anxiety Disorder–This involves recurrent thoughts of developing a physical illness or about having one. Persons with this disorder may or may not have an actual somatic problem. They will look at themselves for signs of illness (e.g., checking skin blemishes over and over). They may be fearful of developing something in the future. Much time is spent worrying about one’s health. This is distressing!

People with Illness Anxiety Disorder and Somatic Symptom Disorder may engage in a behavior known as “doctor shopping.” For instance, a person with Illness Anxiety Disorder may go from doctor to doctor with the expectation that a physician will diagnosis and then treat an illness—thereby confirming in the patients mind that they are sick. If a doctor says they are not sick they may feel some relief, but then worry that a mistake was made, which then prompts them to move on to another doctor with the hope that a diagnosis will be given. Those who engage in such behavior may end up with numerous medications that they simply do not need. They may also be subjected to unnecessary and expensive testing that is unwarranted as well.
You may have heard of Hypochondriasis. This is no longer a DSM-V diagnosis. Those with Hypochondriacal concerns are now diagnosed with either Illness Anxiety Disorder or Somatic Symptom Disorder.
Conversion Disorder
These disorders involve a deficit in some type of physical functioning. For instance, an individual may lose sight in an eye or develop deafness in an ear without adequate medical explanation. These disorders may be psychological in nature, in that a psychological issue could be causing the physical problem. The problem would be considered to be psychogenic in nature. For instance, an individual who lost their sight might be trying not to see some issue in their life. Whoa! That sounds a lot like a Freudian explanation!
Finally, some individuals with this disorder manifest what is called la belle indifference. This is a nonchalant attitude towards one’s symptoms. What this means is that the individual does not seem to be particularly distressed by their symptoms So, an individual who has lost their sight does not seem too concerned!! Yikes! La belle indifference is not a definitive diagnostic test for the illness. It may suggest, that at some level, the patient is not entirely convinced that their own symptoms are real. Think about that one for a little while!!!

Factitious Disorder
This type of disorder involves an individual who purposefully makes oneself or another sick or injured. Those who engage in such deceptive behavior may not even gain any type of reward for such actions.
Here is one you may have heard of: Munchausen’s Syndrome and Munchausen’s Syndrome by Proxy. In Munchausen’s Syndrome one would purposefully do something to themselves to get sick (e.g., ingest a poison). In the By Proxy form a parent might purposefully give their own child a poison to make them ill. This then would result in a trip to the ER for attention and treatment.

Malingering-This involves the outright faking of symptoms to gain some type of tangible reward. The reward obtained is sometimes called secondary gain.
